2. Appointments, Cancellations & Late Arrivals

• Appointments: Patients may request appointments via our website or by phone. Confirmation of appointment times is subject to availability.
• Cancellations: We kindly ask for at least 24 hours’ notice if you need to cancel or reschedule your appointment.
• Late Arrivals: We offer a 10-minute grace period for late arrivals. Arriving more than 10 minutes past your scheduled time may result in a missed appointment and a no-show fee.
• Missed Appointment Fee: A fee of $50 will apply to appointments missed without proper notice.

3. Payments, Insurance & Financial Policies

• Payment at Time of Service: Payment is due at the time services are rendered unless prior arrangements have been made.
• Payment Plans: We offer in-house payment plan options. Please speak with a front desk administrator for more information.
• Insurance: We accept most major dental insurance providers. Patients are responsible for any charges not covered by insurance.
• Third-Party Financing: We do not currently partner with third-party financing providers such as CareCredit.

4. Patient Responsibilities

• New Patient Forms: All new patients must complete medical history and intake forms. Please arrive 15 minutes early to your first appointment to complete this process.
• Minors: Patients under the age of 18 must be accompanied by a parent or legal guardian.
